Abstract Background and aims Recognizing the indispensable role of carotid artery Doppler ultrasonography in diagnosing carotid artery disease and mitigating stroke risk following coronary artery bypass graft (CABG) surgery, this research endeavors to examine the connection between carotid artery stenosis and complications arising after CABG among patients treated at Booali Sina Hospital. Methods This cross-sectional investigation encompassed all individuals scheduled for CABG procedures in 2024, who were evaluated for carotid artery stenosis using Doppler ultrasonography. Comprehensive demographic information and ultrasonographic findings were systematically recorded. Statistical evaluations were carried out, with a significance threshold established at p 0.05. Results The study population exhibited a mean age of 60.98 ± 9.19 years, with males constituting the majority (66.2%). Following CABG, cerebrovascular incidents were observed in only five patients. A notable correlation emerged between the occurrence of post-CABG stroke and the mean thickness of the right intima-media layer (IMT) (p 0.05). In contrast, no other Doppler-derived parameters demonstrated a statistically significant association with stroke events. Conclusions The findings of this study emphasize that demographic variables and Doppler ultrasonography measurements, particularly the right IMT, may serve as potential predictors of stroke risk in the aftermath of CABG surgery. Consequently, it is advisable to incorporate Doppler ultrasonography into routine postoperative assessments for CABG patients. Conflict of interest
