Abstract Background and aims Argentina lacks contemporary nationwide data on stroke-care capacity. We mapped stroke-care centres and quantified access to key acute stroke-system resources, focusing on reperfusion, advanced imaging, tele-stroke and organised stroke unit care. Methods CAMINA was a national, cross-sectional survey conducted in from March to December 2025. Centres were identified via staged outreach, responses were deduplicated and verified, and all sites were geocoded. Descriptive analyses summarised infrastructure, imaging, workforce, reperfusion availability, telemedicine and certification. Results We included 173 centres across 24 jurisdictions (100% geocoded). Core 24/7 resources were common: emergency department 100.0%, emergency laboratory 98.8%, CT 95.4% and ICU 98.3%. Advanced imaging was less frequent (CTA/MRA 60.7%, MRI 46.8%, perfusion imaging 22.5%). An acute ischaemic stroke reperfusion protocol was reported by 75.1% and 24/7 neurology support by 63.0%. IV thrombolysis was available in 73.4% (127/173). Mechanical thrombectomy was available in 37.6% (65/173), and 35.3% (61/173) reported 24/7 availability. Telemedicine for acute stroke was reported by 19.1% (33/173), including 14.5% (25/173) with 24/7 coverage. A 24/7 stroke unit and NeuroICU were reported by 20.8% and 17.3%, and center certification by 12.1%. Five jurisdictions accounted for 70.5% (122/173) of centres; eight jurisdictions reported no thrombectomy-capable centre. Conclusions Nationwide mapping demonstrates heterogeneity and geographic inequities in high-complexity stroke care. This geocoded inventory provides an actionable baseline to guide tiered hub-and-spoke network design, tele-stroke scale-up, and targeted expansion of advanced imaging and EVT outside major metropolitan areas to improve equitable access to reperfusion therapies.
