Abstract Background and aims Malnutrition is prevalent across diseases. This study explored the correlation between Controlling Nutritional Status (CONUT) score and 90-day clinical outcomes in acute ischemic stroke (AIS) patients treated with endovascular treatment (EVT). Methods Patients were selected from the RESCUE-RE registry. The primary outcome was 90-day good prognosis (mRS 0–2), with secondary outcomes including all cause of 90-day death, early neurologic deterioration (END), symptomatic intracranial hemorrhage (sICH) and in-hospital complications. Patients were stratified by CONUT score (low: 0–4; high: 5–12). Cox proportional hazards models calculated HRs and 95% CIs. Results Among 1611 patients (male, 67.6%; Mean age, 63.98±12.16 years), 39.4% (634/1611) of patients had good outcome at 90 days (mRS 0-2). The baseline median NIHSS score was 15(11-19), and 79.07% (1269/1611) patients received full recanalization after EVT. The median CONUT score was 3(2-4). Patients were divided into two groups, low scores group of CONUT score (0-4) and higher scores group (5-12). 1235(76.66%) patients got low CONUT score and 376(23.34%) got high score. Patients in the poor clinical outcome group had higher CONUT score. Compared with High CONUT score (5-12), patients in the low CONUT score group (0-4) had higher rate of good clinical outcome at 90 days, lower rate of 90-day death, early neurological deterioration (END), pneumonia and urinary tract infection during hospitalization. Conclusions High CONUT score may be independently associated with poor 90-day outcomes in AIS-EVT patients, and could serve as a potential valuable prognostic indicator.
