Abstract Background and aims In 2023 a novel interdisciplinary clinic was created in a tertiary stroke centre, led by a specialist physiotherapist, speech and language therapist and Advanced Nurse Practitioner. A structured review is provided to all stroke survivors at 6 months post discharge, using the Greater Manchester Stroke Assessment Tool. Where required, survivors with rehabilitative needs are enrolled in a time-limited course of outpatient rehabilitation. Methods Demographic data, the Greater Manchester Stroke Assessment Tool, and outcomes were collected on all patients seen in the interdisciplinary clinic between 14/03/2023 and 01/09/2025. Results The interdisciplinary clinic reviewed 519 patients between 14/03/2023 and 01/09/2025, of whom 476 had had a stroke. Mean age was 69.4 (± 14.3) and 245 (58.8%) were male. Median Clinical Frailty Score was 3 (IQR 2-4.75) and mean Charlson Comorbidity Index was 3.6 (±2.1). At 6 months post-discharge, 391 (93.8%) of patients reported at least one unmet need in the community. The median number of unmet needs was 5 (IQR 2-7). The commonest unmet needs were fatigue (52.8%), not exercising regularly (44.6%), pain (38.4%) and anxiety (36.1%). Physiotherapy input was needed in 23.8%; speech and language therapy in 13.0% and dietetics in 17.1%. This clinic identifies higher levels of unmet need in this cohort than in previous studies. We intend to use this dataset to identify pre-morbid and index stroke-related risk factors for specific aspects of unmet need. We also intend to examine unscheduled readmissions in this cohort, and identify aspects of unmet need that correlate with increased risk of readmission.
