Abstract Background and aims Carotid body tumours (CBTs) are rare neuroendocrine tumour with the incidence of 1-2 per 100,000 which arise near the carotid bifurcation. They are usually benign, 10% can be malignant. Presentation may vary from asymptomatic to pressure symptoms such as pain, dysphagia, odynophagia, hoarseness, stridor, syncope or if secretory (catecholamines) can cause headache, palpitation, sweating and may also invade the surrounding structures- glossopharyngeal, vagus, hyploglossal nerves and sympathetic chain. Methods We report a case of 63 year old male who presented with dysarthria, left sided limb and facial weakness. His CT head showed only small vessel disease.His MRI head confirmed acute right middle cerebral artery territory infarct which was managed with antiplatelets and atorvastatin. Carotid doppler ultrasound showed no significant stenosis of carotid arteries but there was a non vascular structure between left internal and external carotid artery. CT angiogram confirmed a carotid body tumour of 2 cm in diameter, at the left carotid bifurcation splaying the ICA and ECA . Results Vascular MDT recommended surgical excision of the carotid tumour due to the potential for malignant transformation and risk of pressure symptoms as it enlarges.Vascular surgeon will assess for secretory activity prior to surgery and convey histopathological diagnosis. Genetic testing for succinate dehydrogenase( SDH) mutation will be performed as 10-35% has hereditary predisposition. Conclusions CBTs are slow growing tumour which are identified by clinical examination or found incidentally by imaging studies. Surgical excision remained the gold standard of treatment.
