Abstract Background and aims Both stroke and cancer are leading causes of global morbidity and mortality. However, the risk factors and tumor distribution patterns subsequent to stroke remain underexplored. This study aims to investigate the risk factors for cancer incidence after stroke and to describe the distribution characteristics of tumor types in a cohort of stroke patients. Methods We sifted stroke patients from Ren Ji Hospital registry (2002–2007), with the following exclusion criteria: age 18, hemorrhagic stroke, or prior cancer. Patients were followed annually by telephone or outpatient visit, by the date of death or 31, December, 2023. Cox regression analysis was employed to investigate the interactions between post-stroke tumor occurrence and other variables, as well as to determine which types of tumors were more likely to develop after stroke. Results In the analysis of post-stroke cancer risk factors, male gender (p = 0.058) and older age (p = 0.071) emerged as borderline significant predictors, while current smoking (p = 0.062) and alcohol consumption (p = 0.041) demonstrated independent associations, with alcohol use retaining statistical significance in elderly subgroups. Regarding tumor spectrum, digestive cancers (36 cases, 3.6%) and respiratory cancers (25 cases, 2.5%) were predominant. Cancer incidence peaked within 5 years post-stroke, with significant gender-specific patterns in tumor distribution and lifestyle factors (p = 0.033).
