Musculoskeletal metastases from esophageal carcinoma are rare and can be clinically occult. 18 F-fluorodeoxyglucose ( 18 F-FDG) positron emission tomography/computed tomography (PET/CT) is a powerful imaging modality for detecting metastatic spread, especially when conventional imaging may be negative. This case presents a 63-year-old male with obstructive esophageal adenocarcinoma and extensive musculoskeletal metastases. PET/CT findings directed tissue biopsy while, revealing a rare and aggressive dissemination pattern. This case highlights the diagnostic value of 18 F-FDG PET/CT in characterizing unusual muscular metastatic involvement and underscores the importance of whole-body metabolic imaging in staging, producing the classic “tiger-man” pattern on PET in the context of metastatic esophageal carcinoma.
