Abstract

Abstract Introduction Undertreated obstructive sleep apnea (OSA) worsens epilepsy. Refractory epilepsy patients may require both positive airway pressure (PAP) therapy and responsive neurostimulation (RNS). Because both PAP and RNS devices generate cloud datasets, integrated review offers unique opportunities to optimize the management of comorbid OSA and epilepsy. This review highlights patients in whom cloud-based patient data management services (PDMS) enabled longitudinal monitoring, treatment adjustments, and improved clinical outcomes Methods In an urban medical center with both an Epilepsy Program (National Association of Epilepsy Center NAEC level 4 designation) and a Sleep Medicine Program (Accredited by the AASM- American Academy of Sleep Medicine), 2 cases were identified where dual cloud platforms were being used to manage both Epilepsy and OSA. Results Cloud-based PDMS integration for PAP and RNS enabled real-time monitoring, informed programming decisions, and facilitated coordinated care between sleep medicine and epilepsy teams. Conclusion Dual cloud-enabled data platforms for PAP and RNS devices can significantly enhance longitudinal management of patients with comorbid OSA and refractory epilepsy. Integrating respiratory and neurophysiologic metrics may improve treatment adherence, guide device programming, clarify seizure localization, and ultimately optimize clinical outcomes in this complex patient population.
