Abstract Introduction In April 2025, The New York City (NYC) Council enacted a groundbreaking law mandating access to free obstructive sleep apnea (OSA) screening for New York residents regardless of insurance coverage. We reviewed local policy development and enactment to provide a framework for other policymakers to model similar laws in their constituencies to promote OSA screening and awareness. Methods We conducted an archival analysis of historical records by reviewing NYC Council hearing transcripts and recordings, committee notes, and other legislative materials to understand the process of codifying this bill into law. We also conducted qualitative interviews with two NYC policymakers involved in the bill’s success. Results The sleep apnea screening bill, Int #1047-B was introduced to the Council by Councilmember Abreu in September 2024. The bill was passed in April of 2025 after amendments from the NYC Council Committee of Health. Regarding elements of the successful legislative effort, we found 7 themes: personal experience of legislators, illuminating gaps in public health data on OSA, emphasizing consequences on untreated OSA, making the consequences of OSA personally relevant, obtaining leadership buy-in, and the importance of timing and connections with the press. Our qualitative analysis revealed 3 themes on the legislative components of the sleep apnea screening bill: public education and outreach, ensuring access for all, and legislating an evaluation of the program. Policymakers? also discussed the legislative components of this screening bill, and the drafting process to promote free OSA screening in New York City. The qualitative analysis provided insights on the legislative process with 5 themes: process of drafting, law codification and implementation, incorporating science into the argument, and the political process. Conclusion This work may serve as a model resource for future legislation on OSA screening in other constituencies.
