Cutaneous eyelid tumors are predominantly composed of basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs), with baso-squamous carcinoma being a rare and aggressive subtype. This tumor exhibits features of both BCC and SCC, which may lead to misdiagnosis and inappropriate treatment. We present a case of a 60-year-old male who presented with a nonhealing ulcer and crusting along the left lower eyelid. Initial histopathological analysis suggested BCC, but a subsequent wide excision revealed moderately differentiated SCC with features of baso-squamous carcinoma. Immunohistochemistry confirmed the diagnosis, and PET-CT scan showed no systemic metastasis. The patient underwent eyelid reconstruction and dacryocystectomy, with regular follow-up. Although baso-squamous carcinoma is rare, it should be considered in the differential diagnosis of eyelid tumors due to its aggressive nature and potential for metastasis. Effective management, including histopathological and immunohistochemical analysis, is essential for accurate diagnosis and treatment. This case underscores the importance of documenting rare cases to improve clinical awareness and guide appropriate treatment strategies.
