Abstract Objectives: The objective of this study was to determine early complications at 90 days following bilateral simultaneous total knee arthroplasty and to find out the correlation between various demographic parameters (age, sex, body mass index BMI, and preoperative hemoglobin), Functional Comorbidity Index (FCI), and American Society of Anesthesiologists (ASA) scores with these complications. Materials and Methods: All the patients undergoing bilateral simultaneous total knee arthroplasty over a period of 5 years at a tertiary care hospital were evaluated in the present prospective interventional study. Patients were followed up 90 days postsurgery and assessed for complications, if any. All complications were divided into four broad categories – cardiorespiratory, functional, wound-related, and miscellaneous (delirium, seizures, pyrexia, gastrointestinal, dyselectrolytemia, etc.). They were also classified into major and minor complications. Data analysis was done using Python software. Pearson correlation was calculated. A t -test was run. Gradient boosting rather than logistic regression was used to calculate precision, sensitivity, and F1 score, as the data were of an unbalanced nature. Results: Out of 1121 patients, 271 (24.1%) developed complications at 90 days follow-up, out of which 34 (12.5%) were cardiorespiratory, 140 (51.6%) were functional, 49 (18.1%) were wound-related, and 48 (17.8%) were miscellaneous. There was a slight increased incidence of developing early complications with increasing age ( P = 0.047), BMI, and ASA grade. FCI showed a negative correlation (−0.0339) with early complications at 90 days. Conclusions: Incidence of developing complications within 90 days after bilateral simultaneous total knee arthroplasty is 24.1%. Age, BMI, FCI, and ASA score show weak correlation in the occurrence of complications post total knee arthroplasty; however, these parameters do not show a cause–effect relationship.
