FOLFIRINOX chemotherapy is recommended as first-line treatment for metastatic pancreatic ductal adenocarcinoma (mPDAC). While its efficacy has been well documented in clinical trials, the responses observed have been predominantly partial. To date, only rare cases of complete response (CR) assessed by CT scan have been reported in the literature. We present a case of sustained metabolic CR over 23 months assessed by LAFOV FDG-PET/CT in a 56-year-old male patient with poorly differentiated mPDAC treated with FOLFIRINOX. This case highlights the emerging role of latest generation of digital FDG-PET/CT in assessing the therapeutic efficacy of systemic treatments for solid cancers.
