Hepatocellular carcinoma is a major global health concern with limited treatment options and high recurrence rates. This case study presents a 68-year-old male diagnosed with hepatocellular carcinoma who refused liver transplantation and underwent multiple conventional treatments, including Drug-eluting Bead trans-arterial chemoembolization (DEB TACE), targeted therapy, and radiation therapy. However, due to adverse effects and suspected disease progression, the patient opted to switch to Ayurvedic management. ,It incorporated Ayurvedic formulations, like suvarnamalinivasanta, phalatrikadiguggulu, patola, bhumyamalaki, and bhringaraja to support liver function, enhance immune response, and mitigate tumor progression. Over the course of treatment, the patient demonstrated improvements in clinical and biochemical parameters, including a reductions in alpha-fetoprotein and bilirubin levels, clinically improved liver function, enhanced physical strength, and better quality of life. Radiological assessments indicated disease stabilization with no significant metabolic activity. The case highlights the potential of an integrative approach to improve patient outcomes, particularly in those who decline or are unfit for liver transplantation.
