A BSTRACT Introduction: Organ donation is a significant advancement in modern science that offers hope and saves lives. This study aimed to assess the knowledge, attitude, and practice regarding organ donation among the adult population in Chennai. Methodology: A cross-sectional study was conducted among adult residents of Chennai using a convenient sampling technique. Data were collected from 185 participants using a predesigned semi-structured questionnaire. The questionnaire collected information on socio-demographic characteristics, as well as knowledge, attitude, and practices related to organ donation. Descriptive and inferential statistical analyses were performed using IBM SPSS v26. Results: About 84.9% of the participants were in the age group of 18–30 years. About 70.8% of the respondents knew that there are laws regulating organ donation. Only 15% of the respondents have already registered for organ donation, but 76.8% of the respondents are willing to get registered for organ donation. Among those who are more than 30 years of age, 35.7% of them have registered for organ donation, which is significantly higher ( P = 0.014) than those who are <30 years of age. Conclusion: While awareness and positive attitudes toward organ donation were observed, the registration rate was relatively low. These findings emphasize the need for targeted educational campaigns to increase organ donation registration and promote a culture of organ donation.
