Objective: Left ventricular hypertrophy (LVH) is a common complication of primary hypertension (PH) in children that can lead to serious heart problems. Untreated, LVH can progress to more severe issues like arrhythmias. heart failure, and potentially sudden death. Differences in metabolic and ambulatory blood pressure monitoring (ABPM) profiles between children and adolescents with PH and without LVH have not been systematically reviewed. This study aimed to investigate differences in the clinical phenotype of children and adolescents with PH and LVH compared to those without LVH and to identify potential diagnostic or prognostic markers of LVH in PH. Design and method: The systematic review and meta-analysis of articles reporting metabolic and ABPM profiles of children and adolescents with PH and LVH was conducted. Random effects model was used to calculate and compare mean differences. Results: The search yielded 688 articles of which 16 reports met the inclusion criteria. The total population included 2303 subjects (714 with LVH). PH subjects with LVH had significantly higher body mass index (MD 3.56; 95% CI 2.98, 4.13; p < 0.001) and significantly lower high-density lipoprotein concentration (MD -2.31; 95% CI -4.01, -0.61; p = 0.02) compared to PH subjects without LVH. Conclusions: PH children and adolescents with LVH present different clinical characteristics (increased body mass index and decreased high-density lipoproteins) compared to PH subjects without LVH, irrespective of ABPM profile.
