The triglyceride-glucose (TyG) index is independently associated with both diastolic blood pressure and pulse wave velocity in high-risk men, suggesting a bidirectional link between insulin resistance and arterial stiffness.
Objective: To assess the associations between lipid profile–derived atherogenic indices and pulse wave analysis parameters in male patients with high or very high cardiovascular risk. Design and method: Forty-one high-risk male patients underwent non-invasive pulse wave analysis. Fasting serum total cholesterol, triglycerides, LDL cholesterol, and HDL cholesterol were measured. From these, the atherogenic index of plasma (AIP), atherogenic index (AI), Castelli Risk Index I (CRII), triglyceride–glucose (TyG) index, and non-HDL cholesterol were calculated. Results: Mean diastolic blood pressure (DBP), mean arterial pressure (MAP), and pulse wave velocity (PWV) were 85±11 mmHg, 105±21 mmHg, and 9.34±1.51 m/s, respectively. DBP showed significant positive correlations with AI, AIP, CRII, non HDL cholesterol, triglycerides, and TyG. PWV correlated significantly with TyG, whereas MAP correlated with triglycerides, AIP, and TyG. In multiple regression analyses, TyG was an independent determinant of both DBP and PWV, while DBP and PWV independently predicted TyG, indicating a bidirectional relationship. Conclusions: Metabolic disorders characterized by atherogenic dyslipidemia and insulin resistance–related abnormalities are closely linked to increased arterial stiffness and hemodynamic load in high cardiovascular risk men. A reciprocal amplification loop appears to exist between insulin resistance–related dysmetabolism (captured by TyG), elevated blood pressure, and large artery stiffness. The TyG index emerges as a comprehensive marker integrating metabolic risk, blood pressure burden, and vascular damage. Higher diastolic load clusters with more adverse lipid profiles and metabolic disorders related to insulin-resistance.
