Objective: To evaluate and consolidate the latest research evidence on telmisartan across the hypertension care pathway, with emphasis on benefits beyond blood pressure control and its role along the cardiometabolic continuumDesign and method: The TELIENDURE consensus involved 215 hypertension management experts with more than 5000 cumulative clinical experience years. Participants rated 18 clinical statements via a 5-point Likert scale. Agreement levels were quantified, and weighted mean scores were calculated, with scores >100 predefined as consensus agreement. Key domains included comparative efficacy versus other antihypertensives, tolerability, metabolic and organ-protective effects, role in high-risk populations, and relevance to Indian real-world practice. Data were analyzed using GraphPad v10.6.0. Results: All 18 consensus statements were evaluated, with 16 statements (88.8%) achieving predefined consensus (weighted mean >100). Strong agreement was observed for the evolution of telmisartan evidence from pharmacologic characterization to real-world cardiometabolic application (121.0), formulation advances (126.7), favorable pharmacokinetics enabling individualized dosing (120.9), and superiority or comparability versus other antihypertensives including losartan and enalapril (120.3). Telmisartan-based mono- or combination therapy in Indian patients demonstrated high consensus (125.4), as did fixed-dose and free combinations with chlorthalidone (126.7), cilnidipine (121.6), indapamide (116.2), and metoprolol in high-risk patients (122.9). Pleiotropic and metabolic benefits, including partial PPAR gamma mediated effects and organ protection, achieved consensus (117.7 and 107.2, respectively). Lower agreement was noted for the telmisartan–calcitriol combination (77.6) and its potential role in MASLD (73.9), which did not meet the consensus threshold. The overall mean response scores were: agree: 54±24 (95% CI 42 to 75), strongly agree: 44±31 (95% CI 28 to 60), p<0.0001 Conclusions: The Teliendure consensus affirms telmisartan as a cornerstone therapy in hypertension management, extending beyond blood pressure control toward comprehensive cardiometabolic risk reduction. Its strong evidence base, metabolic precision, pleiotropic organ protection, and adaptability to combination therapy position telmisartan as a leading, evidence-backed ARB across the cardiometabolic continuum in real-world Indian clinical practice.
