Does long-term treatment with a single pill combination of candesartan/amlodipine maintain hypertension control over 3 years in primary care patients?
A single pill combination of candesartan/amlodipine effectively maintains long-term blood pressure control over 3 years in a primary care setting.
Objective: After 3 years we re-assessed the hypertension status of the patients that 3 years before completed a clinical study (GPHTPT) in which a good hypertension control was obtained in 700 previously uncontrolled (TA >= 140/90 mmHg) hypertensive patients with a single pill combination of candesartan/amlodipine (CAN/AMLO) while being followed by family physicians in primary health care. Design and method: Clinical re-assessment - 3 years after - of a sample of n=132 patients ageing 65+9 yrs, 59% women, 56% with dyslipidaemia, 19% diabetics that was representative of the 700 pts that completed the GPHTPT study 3 years before ageing 62+14 yrs, 54% women, 53% with dyslipidaemia and 13% diabetics. The GPHTPT study consisted of 12 weeks treatment only with the combination of candesartan /amlodipine (CAN/AMLO) in daily dosages of 8/5 mg or 16/5 mg or 16/10 mg depending on the clinical severity criterion, that at the end hypertension was controlled (BP<140/90 mm Hg) in 76-91% of the enrolled patients. BP was measured in office according to guidelines. Results: After 3 years, hypertension control was observed in 85% of the patients. Patients maintained therapy with one of the 3 doses of the combination (CAN/AMLO) in 87. 9% (in 81% was the only antihypertensive therapy) and the 8/5 mg dosage remained the most frequently used formulation (43. 9% of patients). A fourth drug was need to be added to the CAN/AMLO combination in 7% of patients. Only 7% of patients switched their CAN/AMLO-based medication to another different regimen. No CV event nor deterioration of biochemical parameters were noticed and there was a tendency to some improvement in urinary albumin excretion Conclusions: Single pill combination of CAN/AMLO-based medication was able to maintain for at least 3 years a good hypertension (85%) control in most of patients who were well controlled 3 years before at the end of a 12 week experimental study.
