Abstract Iliac artery endofibrosis (IAE) is a non-atheromatous vascular disease observed in endurance athletes. A 46-year-old female amateur triathlete presented with exercise-induced claudication of the left leg. The resting ankle-brachial index (ABI) was normal, but the post-exercise ABI decreased on the left. Computed tomography angiography (CTA) showed stenosis of the left common and external iliac artery, and magnetic resonance angiography after exercise demonstrated worsening of the stenosis, which was consistent with IAE. The patient underwent endarterectomy with patch angioplasty via a retroperitoneal approach. Postoperative CTA confirmed resolution of the stenosis. The patient returned to triathlon race without recurrence of symptoms.
