Abstract Introduction Abdominal ectopic pregnancies are rare and associated with increased maternal morbidity and mortality. As the majority are diagnosed early in gestation, there are few reports of management of advanced abdominal ectopic pregnancies. Case A 41‐year‐old gravida 2 para 1 presented with fatigue and a known left ovarian cyst. She was found to have a term, viable abdominal ectopic pregnancy and underwent exploratory laparotomy, abdominal delivery, left oophorectomy, and supracervical hysterectomy. The patient and her infant recovered well and were discharged home in stable condition. Conclusion In this unusual case, multiple imaging modalities were needed to arrive at a diagnosis and plan for optimal surgical management. Interdisciplinary care planning at a well‐resourced referral center allowed for the successful management of this complex case.
