Abstract

Wearable Medical Devices (WMDs) are transforming healthcare by enabling early diagnosis, continuous monitoring, and personalized care through advanced biosensors, data analytics, miniaturization, and wireless connectivity. However, challenges such as data privacy, interoperability, and accuracy persist. Regulatory landscapes complicate progress, with the EU’s MDR emphasizing post-market surveillance and transparency, the US FDA focusing on clinical safety and efficacy through the 510(k) pathway, and India (CDSCO) gradually evolving its framework to balance innovation with public health needs. These divergences hinder global harmonization and market access. The objective of this review is to highlight technological advances, existing challenges, and policy directions to promote innovation and harmonized healthcare integration while comparing the regulatory frameworks of wearable medical devices across the U.S., EU, and India. A comprehensive literature review using PubMed, Elsevier, market reports, and regulatory documents was conducted. Government and industry publications, along with relevant literature, were investigated to compare U.S. (FDA), EU (MDR), and Indian (CDSCO) frameworks regarding safety standards, approval procedures, and post-market surveillance. The analysis highlights advances in wearable technologies, including improved sensors, realtime monitoring, and AI integration. Key challenges include data privacy, device accuracy, and user adherence. Regulatory approaches differ: the U.S. emphasizes innovation with strict oversight, the EU prioritizes safety and risk classification, while India is strengthening compliance.Harmonization of global standards is crucial for broader adoption. This study offers valuable regulatory insights but is limited by secondary data. Strengthening validation, data governance, and ethical collaboration is essential for sustainable, globally harmonized integration of wearable medical devices.
