Abstract A 62-year-old male presented with a 1-year history of reduced vision in both eyes. His best-corrected visual acuity was 6/36 in the right eye (RE) and 6/12 in the left eye (LE). Anterior segment examination revealed immature cataracts in both eyes. Fundus examination and multimodal imaging identified chronic central serous retinopathy (CSC) with subretinal fluid in the RE and adult-onset foveal vitelliform dystrophy (AOFVD) in the LE. The patient underwent micropulse laser treatment in the RE and cataract surgery in the LE. At the 3-month follow-up, the RE vision remained stable at 6/36, whereas the LE improved to 6/6 after phacoemulsification. This case highlights a unique coexistence of chronic CSC and AOFVD, suggesting a potential shared pathophysiology involving choroidal thickening.
