Abstract Proliferative verrucous leukoplakia (PVL) is a multifocal, progressive lesion of the oral mucosa, known for its high transformation rate into oral squamous cell carcinoma. We report the case of a 75-year-old male with PVL and had no history of tobacco or alcohol use. After multiple unsuccessful consultations and treatments, a targeted combination therapy led to the rapid regression of the lesion. This report emphasizes the importance of early identification and patient-specific protocol.
