Case: A 4-year-old boy with neurosarcoidosis developed bilateral equinocavovalgus deformities that significantly altered his gait. Our management included serial short leg stretching casts which corrected the neuromuscular deformity and led to a normal, reciprocal heel-toe gait. Conclusion: To the best of our knowledge, this is the first reported case of severe bilateral equinocavovalgus deformity in a pediatric patient with neurosarcoidosis, successfully treated with serial casting without the need for surgical correction. Correction has remained durable at the 2-year clinic follow-up.
