Learning Points Extremely preterm infants with severe BPD are at risk of severe, multisystem COVID‐19. Limited pulmonary reserve predisposes these infants to rapid respiratory decompensation during infection. Thrombocytopenia and hepatic inflammation, though uncommon in neonates, can occur and may be profound. Laboratory work‐up should extend beyond respiratory markers to include haematological and hepatic monitoring. Antiviral therapy may have a role in selected high‐risk neonates, though evidence remains limited.
