Abstract Background and aims Despite WHO calling to emphasize the need for neurorehabilitation, only a few of the Swedish population with stroke are admitted to specialized inpatient rehabilitation and factors predicting admittance are unknown. Aim: This study aims to identify predictors of admittance for specialized inpatient rehabilitation. Methods This is a Swedish national epidemiological study including 74 849 participants with stroke diagnoses in 2018-2021. The study population covered 90% of the Swedish population with stroke incidences in 2018-2021. Participants admitted to specialized inpatient rehabilitation, were identified from the Swedish registry for rehabilitation medicine. Binary logistic regression was used to identify predictors of admittance. Eligible predictors included patient demographics, stroke type and severity, and health status prior to stroke. Results From 74 849 participants, 2503 (3,3 %) were admitted to specialized inpatient rehabilitation. Mean age of the study population was 75 years (SD: 12,6), and 63 years (SD: 13,4) for participants admitted to specialized inpatient rehabilitation. Predictors of admittance to specialized inpatient rehabilitation were intermediate stroke severity, functional independency prior to stroke, higher educational level and receiving thrombectomy. Conclusions Eligibility for thrombectomy and having a higher educational level predicted admittance to specialized inpatient rehabilitation. These results indicate that the decision on admittance is not only influenced by stroke severity and functional level, but also socioeconomic status.
