Introduction Laparoscopic cholecystectomy is the standard procedure for managing gallstones and its complications. The purpose of this study is to identify the current trends of cholecystectomy in Saudi Arabia as it has scarcely been addressed in literature. Methods The current study was conducted as a cross‐sectional design. A questionnaire was distributed targeting general surgeons at all levels. It assessed three aspects as follows: (1) surgeon’s background, (2) preferred techniques, and (3) intraoperative decisions in case of difficulties. Results A total of 303 surgeons completed the questionnaire. Conventional four‐port laparoscopic cholecystectomy was the preferred approach for 75% of respondents, while only 3% used robotic surgery. Less than half adhered to guideline‐based management of acute cholecystitis, with 25% preferring conservative treatment. Use of intraoperative adjuncts was limited: 40% never used intraoperative cholangiography, over 75% did not use ultrasonography, and 85% did not use ICG imaging. Conclusion Our findings reflect the existing variation in practice among different facilities in Saudi Arabia, with limited use of robotic surgery and intraoperative adjuncts. Further outcome‐based studies are needed to guide the standardization of care.
