Abstract Background and aims This study presents preliminary epidemiological data from the Stroke Registry of Ioannina hospitals for the period April–December 2025, corresponding to the first nine months of data collection. Methods Patients hospitalized in the Neurology and the Internal Medicine Departments of the University General Hospital of Ioannina, as well as the Internal Medicine Department of Ioannina General Hospital, were included. Demographic characteristics, vascular risk factors, and NIH Stroke Scale (NIHSS) scores at admission and discharge were recorded. TOAST classification was used for ischemic stroke (IS) etiology. Results Overall, 224 patients were recorded (55.4% men, mean age 67.1 ± 15.0 years). Of these, 59.4% had IS, 17.0% hemorrhagic stroke, and 18.8% transient ischemic attack. The most common risk factors were arterial hypertension (61.2%) and dyslipidemia (62.1%), followed by smoking (29.5%), diabetes mellitus (23.2%), and alcohol abuse (14.7%). Mean NIHSS at admission was 7, and median modified Rankin Scale (mRS) score was 3. Among 133 patients with IS, 20 (15%) received intravenous thrombolysis; median NIHSS scores before and after thrombolysis were 14,5 and 11, respectively. TOAST classification of IS patients revealed large-artery atherosclerosis (16.5%), cardioembolism (16.5%), small-vessel disease (9.8%) and stroke of undetermined etiology (54.9%); among patients in the latter category, only one (0.8%) completed a sufficient etiological workup during hospitalization. Conclusions Arterial hypertension and dyslipidemia were the most prevalent risk factors for IS. The low rate of TOAST classification completion during hospitalization highlights the need for more comprehensive etiological investigation. Continued data collection and a specialized stroke unit are warranted to optimize stroke care.
